This form is how I get to know you and your dog before our first session together. It covers everything from daily routines and health history to training background and the specific things that are bringing you here today.

Plan for about 20–30 minutes to complete it. You don't need to have perfect answers — honest and approximate is exactly right. If something doesn't apply to your situation, just skip it or say so.

There are no wrong answers here. You're already doing something right just by being this intentional.
